Otogenic Brain Abscess

Abstract

|
Since the advent of many potent antibiotics, intracranial complications of middle ear infection has become dramatically reduced in frequency. Of recent, however, evidences are accumulating to indicate that the otogenic brain abscess shows a definite tendency to revival by one or other reasons.
The present communication is concerned with 3 cases of such brain abscess which the author has been during the past 6 months at St. Mary´ s Hospital.
Of the 3 cases, 2 cases showed abscess to be in the cerebellum, one of which was accompanied by hydrocephalus. Remaining one case had abscess in the left temporal lobe. All 3 cases were studied by injection of contrast medium into either abscess cavity through the fistula (Cases 2 and 3) or ventricle (Case 1).
Except for the patient complicated by hydrocephalus (Case 2), abscesses were satisfactorily controlled and cured by draining the lesion via the mastoid and/or craniectomy. The draining through mastoid proved to be effective particularly in the treatment of temporal lobe abscess. It is interesting that the temporal lobe abscess complicated by the perforation into the ventricular system could also be cured by drainage and antibiotic regimen.
